Sample Letter for Parents (.doc file) This is a sample letter for parents/guardians to prepare and have notarized for the youth under age 18 traveling with a chaperone. This letter is required. If traveling with a group, be sure to have this letter prepared on the church or school letterhead and have the signatures notarized.
Parent Medial Release and Authorization Form (.pdf) This is a required form.
